Antipsychotic medications not effective in ICU patients with delirium
A systematic review and meta-analysis found no improvements in delirium- or coma-free days, mortality, duration of mechanical ventilation, or ICU or hospital length of stay in critically ill patients with delirium who received antipsychotics versus those who did not.

Secondary benefit seen for early tracheotomy in cardiac patients with prolonged mechanical ventilation
Mechanically ventilated patients who had early tracheotomy or prolonged intubation after cardiac surgery didn't differ in number of ventilator-free days or infection rates, but the former required less sedation, a study found.

Noninvasive ventilation options appear to offer benefits over standard oxygen, review finds
Compared to standard oxygen therapy for patients with acute hypoxic respiratory failure, continuous positive airway pressure by helmet probably reduces mortality and high-flow nasal cannula probably reduces the need for invasive mechanical ventilation, according to moderate-certainty evidence.

Reintubation rates lower with noninvasive ventilation or high-flow oxygen than standard therapy
The results of these 2 studies add to the accumulating evidence that noninvasive approaches have a role in reducing respiratory failure and need for reintubation, according to an accompanying editorial.

Inappropriate CAP diagnosis common in hospitalized adults
A prospective cohort study of 48 hospitals in Michigan found that 12% of patients hospitalized with community-acquired pneumonia (CAP) appeared to have been diagnosed incorrectly and that of these, 87.6% received a full course of antibiotics.

RSV, other viruses associated with severe respiratory disease, length of stay in adult inpatients
Hospital patients with respiratory syncytial virus (RSV), human metapneumovirus, or parainfluenza virus were more likely to require noninvasive or invasive ventilation than general medicine patients who did not test positive for any of those viruses, a national study found.
