Active strategy to diagnose PE in COPD inpatients did not improve outcomes

Embedded risk assessment tool increased appropriateness of VTE prophylaxis
Physicians used the optional tool embedded in their order set on only 24% of general medical patients, but the rate of appropriate venous thromboembolism (VTE) prophylaxis nonetheless increased from 43.1% to 48.8%.

Two studies offer insight on using age, d-dimer, algorithms for diagnosing PE
For patients with suspected pulmonary embolism (PE), a meta-analysis found that pretest probability-dependent d-dimer thresholds were the most efficient strategy, while a trial found that the YEARS rule with an age-adjusted d-dimer cutoff was noninferior to a conventional strategy.

Rivaroxaban after discharge may reduce thromboembolic risk in medically ill
An industry-funded trial that gave medical patients rivaroxaban for 45 days after hospital discharge found decreased risk for a composite end point of symptomatic venous thromboembolism, myocardial infarction, nonhemorrhagic stroke, and cardiovascular death compared to placebo.
