Adverse events found in nearly 1 in 4 hospital admissions
In a retrospective cohort study using a random sample of admissions at 11 hospitals in Massachusetts, clinicians judged 22.7% of adverse events to be preventable and 32.3% to be serious or worse.

Harm during hospitalization more common in patients with developmental disability
Hospitalized patients with intellectual disability, chromosomal abnormalities, pervasive developmental disorders, and congenital malformation syndrome had a 2.7-fold higher risk of an avoidable patient safety incident than those without developmental disability, according to an analysis in England.

1 in 5 patients had adverse event after ICU discharge to hospital ward
The most common adverse events after transfer from the ICU were supportive care failures (e.g., falls, pressure ulcers, fluid and electrolyte disorders) and drug-related events, and more than a third were judged to have likely been preventable, a Canadian study found.
