COVID-19 guideline update from NIH, postdischarge anticoagulation, remdesivir effects
The latest news on inpatient COVID-19 care includes some changes to the NIH guidelines for COVID-19 treatment, as well as studies showing that rivaroxaban after discharge reduced clots and remdesivir helped inpatients on no or low-flow oxygen but not those who were sicker.

Conservative oxygen strategy didn't reduce time on ventilator for ICU patients
Mechanically ventilated ICU patients were randomized to usual care or a protocol that included both an alarm when pulse oximetry went to 97% and decreased fraction of inspired oxygen; the groups were compared on outcomes of ventilator-free days and mortality.

Expert panel recommends a maximum oxygen saturation for hospitalized medical patients
The panel from The BMJ chose a maximum saturation target of 96% for most patients on supplemental oxygen and issued recommendations on initiating oxygen in patients with myocardial infarction or stroke.

Mixed results for remdesivir, negative results for canakinumab, and new vaccine information
Remdesivir appeared to be of benefit in one study but was associated with increased length of stay and no improvement in survival in another, an industry-funded trial found no benefit for canakinumab in severe COVID-19, and the FDA noted a possible increased risk for Guillain-Barré syndrome with the Johnson & Johnson (Janssen) vaccine.

New guidelines, authorizations on remdesivir, bamlanivimab, other drugs for COVID-19
The World Health Organization and Infectious Diseases Society of America (IDSA) changed their guidelines to be less favorable to remdesivir. Both the IDSA and NIH expressed reservations about bamlanivimab, while the FDA issued two new emergency use authorizations.
