In a retrospective cohort study using a random sample of admissions at 11 hospitals in Massachusetts, clinicians judged 22.7% of adverse events to be preventable and 32.3% to be serious or worse.

New risk score appears valid for predicting mortality in ED patients with acute heart failure
Use of the MEESSI-AHF score could improve patient and economic outcomes by identifying high-risk patients who can benefit from inpatient therapy and triaging low-risk patients who can be safely discharged from the ED, study authors said.
