A new analysis of the Hospital Readmissions Reduction Program found that hospital revisits within 30 days actually increased among Medicare patients admitted for heart failure, acute myocardial infarction, or pneumonia from 2012 to 2015.

Flexible visitation, ICU diaries have little effect on delirium, PTSD
Allowing ICU visits for up to 12 hours per day did not affect delirium rates versus 4.5 hours or less per day, and ICU diaries completed by family members and clinicians didn't reduce symptoms of post-traumatic stress syndrome (PTSD) compared with usual care.
